Background: Oral rehabilitation in adult patients with complex anatomical and immunological profiles remains a major challenge. Different options exist, but implant rehabilitation may be rendered difficult if no maxillary bone is present, or if a considerable centripedal anatomical situation is present, or if the patient is poly-allergic.
Material and Methods: A patient with severe maxillary atrophy, maxilla centripedal resorption, classe III Angle, maxillary bone reconstruction with free flap, and multiple allergies necessitated oral rehabilitation.
Results: Different therapeutic options were considered, which are detailed in this presentation and quad zygomatic rehabilitation appeared to be the better option.
Conclusion: This case demonstrates that in highly challenging clinical situations, options are chosen considering both medical background and particularly allergic background, and technical possibilities regarding anatomical considerations.
