Summary: recent advances in bioengineering offer promising solutions to overcome the challenges of complex surgical reconstructions. Among these innovations, the development of biomimetic periosteum opens new perspectives due to its ability to mimic the biological functions of natural periosteum, particularly in bone regeneration. This presentation aims to explore the central role of biomimetic periosteum in modern reconstructive approaches. Following an introduction to the biological properties of the periosteum and its key role in tissue healing and regeneration, we will delve into innovative materials designed to replicate its structure and functions. Concrete clinical examples will illustrate its application in combination with 3D implants, hydroxyapatite bone substitutes, and vascularized flaps. Current challenges, such as the standardization of biomaterials and the limitations of bioprinting technologies, will also be addressed. Finally, a forward-looking discussion will focus on the integration of cell therapies and bioactive solutions into the treatments of the future.nt
